Introduction to LEAF.
LEAF was developed at the Univesity of Wisconsin - Milwaukee in 1996 by
Dr. Susan McRoy
and Alfredo Liu-Perez.
LEAF stands for Layman Education and Activation Form. It consists of a
medical history form that customizes itself and the information it provides.

The medical history form gathers information from the user, which it
uses to create a user model. This user model is used to dynamically customize
the medical form itself and the information provided to the user. Users
can ask LEAF to define medical terminology and to provide them with a list
of topics of interest to them. This list is customized to the user. LEAF
is part of a bigger project called PEAS

General Instructions on Using LEAF
Below are general instructions on how to use LEAF

Press Go to to go to a specific questionaire directly.

Press Previous to go to the previous page.

Press Next to go to the next page.

Press Definitions to get the definition of a medical term.

Press Dialogue to get suggested topics.

Press Help to get help on filling the form.

IMPORTANT: Press the RETURN key after typing numbers in the form.

Problems Using LEAF?

If you notice that the information you typed in the medical form disappears
when the browser changes points to a different page, you may need to increase
the amount of cache memory in your browser. Otherwise LEAF will be reloaded
each time the browser comes back to it and previous information in it will
be lost.
